Position: 2nd Shift Nurse (RN/LPN)

Job Description

St. Luke's Hospital Transitional Care Center is currently seeking a compassionate and skilled 2nd Shift Nurse (RN/LPN) to join our team. The ideal candidate will have a strong passion for providing exceptional care to patients in a transitional care setting. This position is specifically for Wednesday & Thursday evenings, providing care and support to patients during the second shift of the day.

Responsibilities

• Assessing and evaluating patient health statuses
• Developing and implementing individualized care plans
• Administering medications and treatments
• Monitoring and documenting patient progress
• Collaborating with healthcare team members

Qualifications

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of Iowa
• Previous experience in a transitional care or similar setting
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Flexibility to work on Wednesday & Thursday evenings
