The Opportunity:
St. Luke's Hospital Transitional Care Center in Cedar Rapids, IA is currently seeking a dedicated LPN/RN to join our team for 1st shift. This position offers a sign-on bonus for the right candidate who is passionate about providing high-quality care to our patients.
Job Responsibilities:
- Providing direct patient care in a transitional care setting under the supervision of a registered nurse or physician
- Assessing patients' needs, developing care plans, and implementing treatments
- Administering medications and treatments as prescribed
- Monitoring and documenting patients' progress and vital signs
- Collaborating with interdisciplinary team members to provide comprehensive care
- Ensuring compliance with all regulatory requirements and best practices
Qualifications:
- Valid LPN or RN license in the state of Iowa
- Previous experience in a skilled nursing or transitional care setting preferred
- Excellent communication and interpersonal skills
- Ability to prioritize and multitask in a fast-paced environment
- Strong attention to detail and critical thinking skills
- Commitment to providing compassionate and patient-centered care
